probably a squib (under-powered) load. It lodges in the barrel. Shooter pays no heed to the odd happenings at High Pressure at the end of his arm fires 4 more times (5 rounds is standard for a single action revolver). Each bullet lodges behind the first and the gasses exit the cylinder gap in an incredibly large fireball. After firing he looks at the target and sees no holes. Then someone checks the bore...
