The book being the necronomicon and sprouting tentacles, locomotion would just be a matter of opening a crack between pages 126 and 127. Reading the text itself sends people mad so all you'd need to do is attach the book to their face at the relevant page: (once more: tentacles!)

Improved grab: a must.

Call this a supernatural ability: At will, confusion after a successful grapple check. Becomes permanent if the book stays attached for a number of rounds equal to the target's wisdom score. Will save of 20 required to resist cumulative for each round.

Constrict: once attached it can use the assassin vine's constrict ability to deal damage.

Can also cast summon spells to protect itself: consisting of level-appropriate aberrations/demons/fiends/devils/undead (depending on the flavour of the book) but they will consider everything but the book to be a threat.
