My assumption is that it is his avatar that will feel no pain. So, perhaps the doc should have said meta-virtually painless. But then, in the doctor's reality, virtual would mean Sid's avatar. After all, the doctor is not in our reality, but Illiad's cartoon reality. Therefore, virtual, spoken by him, means avatars of those present within his reality construct.
Sid will feel pain!
