In recent times, the Gamma Knife from Elekta has had some software malfunctions that resulted in the emergency stop not working. I don't think there were any fatalities, but there were injuries. Of course, that's just poor design. It should not be possible to override an emergency stop in software. It should simply cut power to the series of solenoids keeping a group of spring-loaded radiation shields open, and at the same time activate the patient retraction motor.
