I usually identify myself and company if I'm calling in a professional capacity. For personal calls, I usually just ask to speak with $NAME. My own philosophy about personal calls is if you're not $NAME, then it's not really your business who I am or why I'm calling $NAME. Of course, it helps that $NAME is usually a friend expecting my call or at least knows who I am...
