It turns your PC into a bot, capable of doing whatever the malware that it downloads will let it. That is potentially anything at all, including (but not limited to) key logging, spam sending, explorer hijacking, file snooping and transmitting.

There are tools out there to detect it; nmap even has an update that allows it to scan a local network and detect it on other PC's (conficker is capable of deactivating anti-virus programs). Or, so I'm told.
