Letters to our previous presidents are addressed to "President... " and they are still referred to as "Mr President".

Without looking at the way in which it was delivered or addressed (as in the envelope/package, there's no way to determine who was at fault. Obama and his administration may have had nothing to do with it.

It may have been properly written and addressed, and just mis-delivered by whoever delivered it. As it was aimed at Chirac, and seemingly correctly so, it's unlikely it was hand-delivered, and only possibly sent as part of a diplomatic pouch.
