I tend to think of the skin as outside. I think I would consider most coverings as outside even if you were within the thickness of the covering. Maybe it's the barrier function of outside. Light that penetrates through some skin cells before stopping would still be outside.

Exposure of normally inside parts wouldn't mean I always treat them as outside. If I can see your insides, something is horribly wrong. It may move parts outside but they wouldn't be the outside.

With the brick, the thickness of the outside isn't terribly relevant for me. Parts within the manufactured surface being exposed wouldn't matter much either unless it changed the dimensions of the brick enough to be a problem while building. What is outside might be more closely tied to what is a brick. If the brick is broken enough that I can't use it, it's not a brick.
