correlate /proc/scsi/scsi with the fdisk output, you might be able to figure it out that way. Also have a look at /proc/mounts, it will show you all the active mounts, not just the ones in fstab.

Now, if the LUN is directly mapped to a VM, it might not even show up in the control domain.
