      Black level is the lightest value that's still by hobbs 2008-05-05 13:16:32
considered black; white value is the darkest value that's still considered white; anything in between is considered neutral. The best way to understand their effect is to fiddle with them. Yeah, you want a small radius, probably 2, and I would try with both of the checkboxes unchecked to begin. Adaptive does some sort of on-the-fly adjustment to the threshold but I don't know exactly what; recursive does multiple runs of the despeckle, which you probably don't want.
