A doctor would be expected to weigh all the consequences of his choice. Many ethical choices are affected by considerations of possible personal consequences or consequences for the community.

For instance due to malpractice suits, many US physicians are getting out of obstetrics. Many towns are now left with no specialists in the area of prenatal care and childbirth. I would argue that in such a situation a doctor would have to weigh the consequences to the town of losing its only obstetrician.

In this case, the doctor has been given a Sophie's Choice for which there is no clear way out.
