Yes, medicine is complicated that a lay person cannot figure out what the pros and cons are of any given course of action. That, I agree, is the doctor's job. But once a doctor figures this out and explains it, it's up to the patient to decide which course of action to take.

Let me give you a hypothetical scenario. You get some fatal, hideously painful disease. There are two options. Drug 1 will remove the pain, but will not extend your lifespan by more than a year or so. Drug 2 may cure the disease, or may kill you. Do you really think it's the doctor's call which drug you take?

I would want to make that decision myself. I think it would be my responsibility. The drugs have different goals. The doctor's job is to pick the means; the patient has to pick the goals.
