Scribble something illegible on their charts, then blame the nursing staff if something goes horribly worng...

Ethically, I guess you give each child a different vaccine.
That way you eliminate the non-working vaccine (and thus be able to make the "right" decision from that point forward) without the potential to loose both children.

Glad I'm NOT a doctor. Triage is a B5.
