Everyone should have had their lights on but the conditions were such that it wasn't automatically suicidally unsafe not to have lights on. Cars that aren't fog coloured are visible from a reasonable distance even without lights. In really bad fog people realise that they can't see well and will (hopefully) take that into consideration. The conditions this morning were such that some people may think that they can see fairly well because most cars are visible from a reasonable distance even without lights. Basically the worst possible conditions to be invisible in. And due to the trend of having colour matched bumpers certain cars in certain colours give you no contrast or clear edge for your eyes to latch on to.
