If you are at a place where you feel fine, respected, loved, cared for, and it is the strongest feeling, and you don't want to go somewhere else (except for vacation or similar) - that's home.
If you are somewhere in the world and feel the urge to go somewhere else where you've been before and where you feel like in the first sentence, you may call that place home.

For me it is quite easy: I've got a city where I've been born, where my parents live. But it is not the place where I feel easy, respected, calm. After university I moved to another city (Leipzig, Saxony) where a good friend lives (now my best friend) and where I got my job - and I now call Leipzig my adopted hometown. It feels much easier - the people are more open minded, tolerant, curious and friendly. There's a reason why Leipzig is one of the worlds most important cities of Gothic (, Metal, New Wave) subculture.
