I'll use Parker v. DC in this example as it's a decision that is both current and has been discussed here. Currently the District Court has ruled a law unconstitutional if that ruling were not being appealed that ruling would be law for that district only. If the SCOTUS declines the case it remains law for that district. If the SCOTUS takes the case their ruling is the law of the entire United States.
