1. XHTML 1.0 Transitional most of the time (The project at work is HTML 4 Transitional because we needed to use IFrames which don't exist in XHTML)
2. I always try to make them validate perfectly using the validome html and css validator
3. Semantic, with the CSS doing all the styling
4. CSS layouts unless I really need to use table layouts for something (very strict layouts that CSS can't quite manage for example)
