(and no I don't code for a living, but I do have a coders background, and I'm a tester, so I have to ensure that what you code fits the requirements that the clients have written down)
Basically you have this:
Client writes down some requirements, with some sort of vision of how it's going to be.
Programmers and designers pick that up, and build the application to fulfill those requirements.
Client sees the end-result and complains because it doesn't fit his/her mental view of how it was supposed to be.
At least I've seen such things happen.
Most times it happened because of a lack of communication between clients and the design team. Which causes inconsistencies and misunderstanding of what exactly is required.
