Sanity's Requiem. Love that game. And BSOD is one of many weird effects that occur when a character's low on sanity. There's a fake no controller connected screen, a fly running around that's made to look like it's on the inside of the screen, several odd cutscenes with ghosts and the like, and other strange effects that reset after they've played out, like the character's head, limbs, and torso exploding in sequence, while you continue to make them walk around.
