Once it leaves your hands, it's no longer carry-on. When I travel, I always assume my luggage will be lost, and pack my carry-on accordingly. I've never had problems with direct flights. Layovers on the other hand, my luck has been less than stellar. Usually due to ground delays shortening my layover time to the point where I'm sprinting between gates to make my next flight.
