In all the other examples you gave, the risks are a neccessary part of the job. In a bar, there's no reason the staff have to be exposed to cigarrette smoke, and it's easy to eliminate by banishing those poisoning their environment with carcinogens to somewhere where they won't affect other people.

Also, in those other jobs, protective equipment can be work to alleviate risks. I don't think gas masks on bar staff would go down well. ;)
