That depends on what I'm doing. I use my fingertips for more precise control, the middle segments for large movements (like left side of left screen to right side of right screen), or even just spin the ball to move the cursor quickly to the general area I want it in. I seem to have my middle and ring fingers on the ball in most cases.

In any case, I've got the heel of my hand on the desk with my hand arched so my palm is clear of the mouse. That might sound a bit uncomfortable, but it's actually quite close my hand's relaxed position.

I normally use thumb and ring finger on the buttons, unless I'm doing a right-click and drag. If it's been a long day of much clicking, I'll sometimes use my index finger instead of thumb on the button.

Hopefully, some of this will make sense and be of help.
