the buttons and my index and middle finger for moving the ball.
Sometimes when moving a long way to the side my ring finger will be on the ball but normally it is resting by the side of it. Other times my ring finger will be over the right button and I'll use both fingers to click it.

Generally the ball rests so that the first knuckle of my middle finger is roughly on the centre line and just forward of the top of the ball.
The base of my plam is just on the back of the mouse and then I have a wrist rest anywhere from 2-5 inches further back (so it's under my arm rather than my wrist).

When moving the ball I'll normally use everything from my finger tips to the second knuckle, just whatever ends up on the ball depending on where I'm moving it to. The one exception is that I'll tend to use just my finger tips and arch my fingers more if I need very fine control.

The thing that I found took the longest to get used to was right dragging things. I still tend to resort to my ring finger for the button in that situation.

Also the ball will tend to free up and move a little smother after you've been using it for a few weeks.
