by that child's parent, guardian, older sibling, grandparent, perhaps aunt or uncle. To me, it has connotations of familial affection, and is completely asexual, so gender doesn't factor into it. Tallness isn't really a factor; it's just coincidental, because children aren't fully grown yet. I'd never expect anyone to kiss me on the forehead. That might be because, in general, I tend to expect nothing from anyone, and when I do get something nice from someone, I'm pleasantly surprised. :)
