The ointment is very oily and can block the pores in your ear, and that can start up a sebaceous cyst. That's even more painful. The cream is a bit better at preventing that. Also try to get the cream that has lidocaine in it - it'll cut down on some of the surface pain if there is any. You may need systemic antibiotics, though, if you're running a fever. I used to have infections all the time, and I found that ear drops + Septra beat out just ear drops every time.
