Openwiki

First, pick which wiki you want to run (I listed 3, but there are dozens) based upon what you like and what language they use. (Openwiki is ASP for example, so don't run it on a apache unless you want headaches.)

They will often have a list of different databases they support, as well as the "default" empty database or script to create it.

I mostly run Openwiki on the windows machines, and Mediawiki on the freebsd boxes that I actually run a wiki on.
