And you need this in fstab:
none /dev/shm tmpfs defaults 0 0

Other then that I can't quickly find anything in the udev config files which would cause udev to create /dev/shm. IOW, I just think that the fact of it being compiled into the kernel, and it being mounted is enough to create it. Not sure if this is true though so YMMV.
