NASA is about to let managers overrule engineers in order to meet a launch schedule. clicky

"Some members of NASA's safety office said the shuttle shouldn't be allowed to fly until more foam is removed. However, they were overruled by their managers, who argued they had taken all the precautions necessary to guarantee a safe flight."

I think that each time a manager tries to overrule on a safety issue they should be asked to view the Challenger "accident".
