OK - so we're doing some battery maintenance on this sucker. It's got a large "fuse safety thingy" (about the size of a candy bar with a metal contact lug at each end - ceramic material in the middle) that you insert and remove with a special tool. It comes in-line before the final output isolators. SOMEONE flipped the isolators to "live". When I put the fuse thingy back in, there were about 30 pcs and an old monster of a Wang Mini (with 3 x 288mb drives the size of a chest freezer each) all switched on, waiting for juice.... The fuse thingy literally exploded. I was left standing there looking very stupidly at the remains of the insertion tool and picking bits of copper and ceramic chips out my hair. I guess the initial load was a little high.
