As the person trusted to carry out a person's wishes.

My father-in-law had a medical mishap (short version, the anesthesiologist for an operation he was supposed to have messed up big-time leaving him in a coma before the operation could even be started. Thanks to Nevada law the SOB is still practicing medicine and the family has almost no recourse but I degress). He had a living will. The whole family knew he wouldn't have wanted to "live" like that. His wife refused to let him die and kept him alive in the coma for months because she'd "rather have him a vegetable then live without him". Because she was the his trustee her word was law. That, and her actions after his eventual death, tore the family apart.
