It was staged: It did happen, but the day before. The photographer didn't want to stick his neck out to snap the picture, so he had some marines recreate it the next day, after the battle was fought. Actually, I think two of the guys are actually sailors, not marines. I can't remember where I read this. It was dead-tree version, so I can't provide linkies. It's still an impressive photo.
