with it being on the back of the surge supressor. This isn't the same location that blew out the rest though is it? Ie: Have they all died in this configuration, or were they before the supressor previously?

Honestly, I'd call and report the problem to your phone company, especially if you have the 'dead' filters still.

I've forgotten to put on filters sometimes and haven't had an issue, nor have I ever had a filter die. I'd question if the line quality is okay in your residence.

Just my $0.02 worth.
