The issue genetically is with widespread cousin marriage which could lead to inbred genetic issues like some of thr royal dynasties face after centuries of the practice of close familial marriages.

My wife's Parwents' friends are cousins whio are married. They lived on opposite sides of the country, met as adults, fell in love, married in Nevada, and have 2 adult children. They did genetuic counselling and have not hid the fact from their friends. It IS a wierd feeling to see pictures of the three sets of grand parents on the mantel :-) but as the linked article mentions, the risks are not statistically a dramatic increase compared to the overall risks.
