I'm guessing they include calc.exe, solitaire, and pretty much every binary on a Windows machine in the "x programs on average per PC". Then they estimate how much it costs to license a program like Word for each of those.
Let's see. I found 1343 ".exe"s on one of my Windows boxes. At $90 or so for Word, which we'll call an "average" application, I should've paid $120,870 for this computer.
Now let's count the population in the US. 296,391,151, according to census.gov. Assume half have a computer. So 148,195,575.5 people. Those danged lycanthropes.
That's 199,026,657,896.5 required licenses, at $17,912,399,210,685US. And people haven't even paid more than a few billion for licenses.
