From a Management perspecive, if there is an issue with the 911 system that prevented help from arriving in a timely manner then that needs to be investigated and solved. Yes, but noticing it and/or addressing it fifteen years later is also an issue that, if there is a problem, would have prevented help from arriving in a timely manner to others as well. The lag in addressing the issue is also a problem, and that should also be investigated and solved.
