In 99.9% of cases, the disabling and the 'locking' of the users account is the punishment, and they can come back with a different username if they choose to. I've said this many, many times yet people keep ignoring it.

A 'banning' is quite different, and often involves actually routing them (or their entire ISP) into a blackhole. That's used in very rare circumstances.

In the case that someone is disabled for their bad behaviour and they come back as a different username and continue that bad behaviour, yet blackholing their ISP would have a greater negative impact than necessary (ie. blackholing AOL), we have to make decisions as the specific situation warrants. Usually that just involves the moderators disabling that user ASAP.
