Point 1 and 4 are covered for me by "try and be a nice guy", using just plain common sense instead of some arcane rules.

Point 2 is one I deem as stupid because
a) It's awkward and unpractical(that thing would never stay on my lap for more than a couple of minutes anyway)
b) I really can't see the disgust factor in using the napkin above the table
That doesn't mean I'd find people silly for doing so, only for maintaining that you're supposed to.

Point 3, well, I'm for free choice of weapons ;)

Stuff from the test I took went from things like "In what order should you greet other guests?" (plain stupid IMHO, just goes to massage the egos of higher-ups) or "What greeting formula should you use when introduced to $person?" (wtf? I'll never make a good diplomat, but who comes up with that kind of rules?) over "In what order should you take food from a buffet" and "Do you reuse your plate/cutlery when going to the buffet a second time?" (answers being both senseless and impractical) to questins similar to the TLP: "How should you eat the bread from the couvert plate?" (However I like it best, dangit :P)
