The place I used to work called me up about a month ago saying "We lost all of the /dev directory and don't know what happened." This was a Linux machine using a 2.4 series kernel. After talking some people through some tests I found something interesting. One of the developers had updated parts of the kernel. One of the "updates" hosed devfs. This is why the dev team should never have root access to a live system.
