and can be combined with other procedures.
However, try combining two unrelated procedures within the same office visit and time and you will see the claim rejected real fast.
Granted I haven't been doing medical billing related work for some time now (hmm a decade ago - where did the time go?), but i did work in the field for 10 years. I have seen personal bills more recently rejected as well - yeah my baby's pediatrician ended up having to shoulder part of a bill for trying to be a good doctor and doing (and billing ) two things in one visit (we helped mitigate bill though)
