For taxes, your employer is required to give you a 1099 form if you perform any meaningful amount of work for them as an "independent contractor".

As far as billing, I work for a public safety consulting firm, and we keep track of the amount of time worked daily, split among the various projects worked on that day (in quarter-hour increments) and detail what was done during that time, as well as "non-billable" work that we perform for our company.
