Write on the next *unopened* one the words "No Longer At This Address. Return To Sender." It costs them extra postage to receive it back.
You might have to do it two or three times before they remove your address, depending on how much of that extra cost they can afford. In Australia, it usually works after one. Once, it didn't work after three, so I rang them up: as far as I could determine, they'd completely bollixed some guy's mailing address for his mobile phone account. The operator actually sounded shocked it had happened, but couldn't answer why it hadn't been fixed when the statements had been returned...
