"United States Armed Forces Medal of Honor".
Because the issuance of the Medal is subject to review by a military/congressional commitee, it is often referred to as the "Congressional Medal of Honor". The Military has become much more relaxed, in recent years, about what to call the award, and has even referred to it as the 'CMH'.

An interesting note on CMH recipients: virtually 70% receive it posthumously. It is not ligthly given, or received.
