Unfortunately some cases will go unpunished to the levels that they should. I, personally, detest Donald Rumsfeld and think that on many levels he needs to be held responsible for Abu Ghairab. (sp??)
However, there are many other organizations that are tightly compacted like the military that have similar problems. Colleges, institutions, etc. However, the person who turns the blind eye in those cases or maybe incites it through sexual harrassment is just as at fault as the Lieutenant Colonel that turns a blind eye in Iraq. It does not mean, however, that the University is at fault for what the Football coach ignores...
Neither is the US Military - which as a whole is 100% against it.
