Do your best to learn and not bury yourself in remorse. All you can do is remember and learn from it.

I was once wrestling with a kid (I was about 12), and threw him down on a mattress really hard. The youth home was recarpeting the floors, and we had bare concrete in that room. The kid bounced, hit the floor, and immediately he swelled up just like you said. I was freaking out about it, but his skull fracture turned out OK (a couple of weeks in the hospital, though.)

The really ironic part was that a few days later my appendix burst and I was put in the hospital. Guess who they put me with? I spent the few days until he got out in immense pain (from the emergency surgery) and mortal fear (I thought he'd smother me while I was sleeping.).

All you can learn from that is that these things will happen no matter WHAT you do, and you have to find your way thru the guilty feelings that you really didn't earn. Be well, and your son as well.
