That basis of that "knowledge" was several sources within the intelligence community, both ours and others. Most of these have not been, nor will be, revealed by the intelligence community. It would be presumptuous of you to assume that you know all of the sources the intelligence community has.

And yes, you CAN hide an active weapons program that well. Several countries already have, in fact, and have since become nuclear powers. It really doesn't take that many workers to manufacture the weaponry in any useful quantity or to test it. It really only takes a few to manufacture a nuclear bomb and the materials are actually quite readily available. Biological warfare materials are also easily manufactured by a few people in very small laboratories.

Given the repeatability of manufacturing process related to nuclear and biological weapons, no real testing is required.
