You just have to have a really big forklift. ;-)

They still do it now for some houses. They disconnect the supporting walls from the foundation and jack the building up a few inches then insert a few support beams. The only tricky part is moving. You have to move the house very, very slowly so it doesn't rip the walls apart.

Apparenly, when the flooded the Seaway some of the houses were simply slid across the ice. Check here if you want to read more about the St. Lawrence Seaway.
