Here, I've dredged up a couple of references:
One : Search for 'castration', read the entire paragraph, he's clearly referring to the removal of a phallic symbol, not a testicular one.
Deux : in french, but the summary here is quite clear that a fetish is a replacement for a castrated penis.

I'll post some more as I come across them.
