just input types of radio buttons then the calculation (done via javascript most likely.) For a utility to do it, look a ultraedit, or the built in composer that comes with mozilla. (those are windows utils)

I generally use Dreamweaver for my WebDev but that is overkill, even for what I do (php stuff, layout is NOT my strong point) but it works nice, it's also not free.
