Somehow (wishful thinking?) I missed the 'terminal' aspect.

Modify my original suggestions somewhat, but in general, they stay the same.

1) I still think E is more trouble than she's worth in the current setup... talk to her *frankly*.

2) What will happen if your Mom ever finds out that 'everybody' knew N's real condition except her? Will it be worse if she learns now (with appropriate help - therapist? clergy? etc.?) or if she learns after the fact? (will she have regrets over things not said, no goodbye, etc.?)

3) Have *you* seen anybody for help through this? It sounds like the only person who has any clue as to what you're going through is N. You've taken on one heck of an emotional/psychological load... make sure that you have the support *you* need to get through things now and after.

Damn. It's things like this that make me kind of glad that the deaths in my family (father, uncle) have been of the "healthy one minute, gone the next" variety.

Take heart. It sounds like you are a godsend to your sister and when all is said and done, you'll know you did your best and possibly more importantly, *she'll* know you loved her and did your best.
