they ARE carrying their own fireplace, close to the skin. How come you think it's cold for them? Otoh: nicotine has a very bad effect on veins, narrowing them very much. This way their blood will NOT warm up their skin (though their fire may). But not loosing your body heat by not-heating your skin is THE BEST way to keep the vital parts warm: look to polar bears, polar foxes and the like: they're BUILT to do just that.
